PREPARE FOR YOUR VISIT

New Patients

  •   Complete your registration and medical history forms before your visit
  •   Be prepared to provide your medical history, medications, allergies, previous surgeries, and current symptoms. 

What to Bring

  •  Photo ID
  •  Insurance card(s)
  •  Current medication list
  •  Relevant medical records/testing
  •  Referral or authorization if required
  •  Any questions you want to discuss with your provider 

Arrival Time

  • Please arrive 15 minutes before your scheduled appointment to allow time for check-in and necessary paperwork. Late arrivals may need to be rescheduled. 

Insurance & Payment

  •  Bring your current insurance card(s) and photo ID
  •   Copayments, deductibles, coinsurance, and other patient-responsible balances may be due at the time of service

Testing & Medical Records

  • If you've recently had imaging, laboratory testing, pathology, or other urologic testing performed elsewhere, please make sure the results are available to Brevard Urology Care before your appointment whenever possible. 

Medications

  • Continue your medications as prescribed unless your healthcare provider has given you different instructions. If you're scheduled for a procedure, follow the specific preparation instructions you were given. 

Canceling or Rescheduling

  •  If you cannot keep your appointment, please contact our office as soon as possible. See our Cancellation & No-Show Policy for additional information.
